What is it?

Biologics and topicals represent two major classes of dermatologic treatments. Topicals include creams, ointments, gels, foams, or lotions applied directly to the skin. They are commonly used to treat localized skin conditions such as acne, eczema, psoriasis, rosacea, contact dermatitis, and fungal infections. Topicals may contain corticosteroids, antibiotics, antifungals, retinoids, or immunomodulators depending on the diagnosis. These treatments work by reducing inflammation, clearing infection, or repairing the skin barrier.

What to expect

Biologics are advanced medications derived from living organisms that target specific components of the immune system. They are typically used for more moderate-to-severe chronic skin conditions, such as plaque psoriasis, atopic dermatitis, or hidradenitis suppurativa, when other therapies have not worked. Biologics are usually administered via injection or infusion and work by blocking immune pathways involved in inflammation.


Your dermatologist will recommend topicals for milder or localized conditions and biologics for widespread or treatment-resistant inflammatory diseases. Patients prescribed biologics undergo lab testing before starting therapy and may require ongoing monitoring to ensure safety and effectiveness. Side effects are rare but may include injection site irritation or immune system suppression. Topicals typically have fewer risks and are safe for long-term use under medical supervision.
